Enrollment Applications
The LVCHS application consists of three separate sections as follows. Please read each document carefully.
The first section includes information about the school and should be read thoroughly. While you do not have to return these documents to the school with your application, the application makes reference to many of these documents and asks that you sign to indicate that you have read and understand them.
The second section is the main application, including commitment forms. Some of the pages will require signatures by either the student or parent(s). Please check to be sure that there are no missing signatures before you return this packet to the school.
The last section contains a number of supplemental forms, most of which require a signature.
In addition to these forms, any new application must also include a copy of the student’s most recent report card (for new 9th graders), or an unofficial copy of the student’s transcript, i.e. one without the school seal (for students transferring from another high school).
